Many older adults living in affordable housing communities are managing health concerns, limited income, and changes in mobility. Tasks that were once simple—preparing meals, keeping up with appointments, or managing paperwork—can become harder over time.

Senior Empowerment connects residents with practical support and one-on-one guidance from a Life Coach. Because services are based within the community, help is close by when it’s needed—whether that means navigating benefits, accessing food, or connecting to care.

The goal is simple: help seniors stay in the homes they know, with the support that allows them to live independently for as long as possible.

For older adults, independence can be shaped by small, everyday factors. Missing a medical appointment, losing access to benefits, or struggling with daily tasks can gradually make it harder to remain at home.

Without support, these challenges can build over time, leading to avoidable health complications or the need for more intensive care. With the right help in place, many of these risks can be reduced.

Phoenix Family’s approach focuses on stability—making sure seniors have consistent access to food, healthcare, and support. When those basics are in place, older adults are better able to maintain their independence and remain connected to their community.

A resident’s story

Staying in the place she calls home

Helen had been part of the community for more than a decade, known for her smile and the way she always had something kind—or funny—to say. Then things started to change. She stopped coming to events, struggled to manage her apartment, and missed important paperwork, including renewing her Medicaid benefits. Her family began to worry it might be time for her to leave her home.

Staff at Phoenix Family encouraged Helen to see her doctor and learned that she had experienced several small strokes and was also dealing with depression. With the right support, things began to turn. Phoenix Family helped connect her to Meals on Wheels, a homemaker’s assistant, and worked with her to restore her benefits. With consistent care and support close to home, Helen began to stabilize. Today, she is feeling more like herself again and says she finally has her “ducks in a row.”
